Job Duties & Skills Required:

What You Will Do

The Provider Relations Specialist is responsible for developing, strengthening, and maintaining positive contractual relationships with service providers to ensure an adequate, high-quality PACE provider network. This role supports network growth and stability by conducting quality reviews, coordinating provider training, managing communication, and serving as a liaison between contracted vendors and internal PACE teams. This role collaborates closely with the Provider Network Manager, Provider Network Business Analyst, clinic schedulers, and the interdisciplinary team (IDT) to ensure provider performance meets regulatory and organizational standards. Supports and lives out Immanuels Mission and CHRIST Promises.

Key Areas

Key Responsibilities and Duties of the Job

Provider Network Development & Relationship Management

  • Develop and maintain strong, positive relationships with contracted providerssuch as nursing homes, assisted living facilities, and specialty service vendorsto ensure a robust PACE provider network.
  • Identify, cultivate, and onboard new providers/services needed to support network expansion and continuity.
  • Prioritize and manage communication with contracted providers following established tier-based frequency guidelines.
  • Provide, at minimum, quarterly touchpoints with all contracted vendors, offering ongoing education, updates, and support.
  • Collaborate with network leadership to identify providers with strained relationships and strengthen partnerships with high-value or essential vendors.

Quality Oversight & Annual Reviews

  • Conduct on-site annual quality reviews of all contracted nursing homes and assisted living facilities.
  • Assist the Provider Network Business Analyst in ensuring audit checklists are completed after contract implementation.

Training & Education

  • Deliver initial orientation and onboarding training to all newly contracted providers.
  • Provide updated PACE training materials at least annually to contracted providers and staff to support ongoing education.

Communication & Issue Resolution

  • Serve as a liaison between contracted providers and the organization to resolve concerns promptly and effectively.
  • Coordinate internal meetings with key stakeholders to address provider issues and communicate final action plans back to contracted providers.
  • Communicate vendor status changes, contract revisions, and network needs to leadership monthly in collaboration with the Provider Network Manager.

Collaboration & Community Engagement

  • Participate in monthly internal provider meetings and weekly contracted services meetings.
  • Attend community networking events, speaking engagements, and special events to represent the organization.

Other

  • Performs other duties as assigned or requested.

Skills & Requirements

Education-

  • Bachelors degree in healthcare administration, social services, business, or related field is required.
  • Equivalent years of experience may substitute for education requirement.

Experience-

  • Experience in provider relations, network development, contract management, or care coordination, preferred.
  • Knowledge of regulatory requirements for PACE and long-term care, preferred.
  • Equivalent years of education may substitute for experience requirements.

Other Requirements-

  • Must have a valid drivers license, proof of insurance and have means of transportation.
  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).

KSA- Knowledge Skills and Abilities-

  • Knowledge of computer applications including Outlook, Word, Excel, database software.
  • Strong communication, relationship-building, and organizational skills.
  • Skilled in written and verbal communication.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
  • Skilled in follow-up and follow-through with people and appointments.
  • Ability to project professionalism, approachability, and genuine interest to various audiences.
  • Ability to speak to groups of people with ease and to communicate clearly and effectively.
  • Ability to work independently with minimal supervision.
