Position Summary
The MMI Care Coordination Lead Social Worker provides direct and consultative services within the Care Coordination Program to medically and socially complex patients and their caregivers receiving intensive specialty services at MMI. This role involves assessing psychological and social needs, collaborating with healthcare professionals to develop treatment plans, and providing counseling and therapy to help patients and families navigate the emotional and social impacts of illness and disability. The position is part of an interdisciplinary team that includes social workers, behavioral health providers, interpreters, medical providers, and parent resource coordinators.

Required Additional Knowledge, Skills and Abilities:
1. Strong organizational skills with the ability to work independently and to prioritize workload and set deadlines.
2. Excellent communication skills necessary (oral and written). Must be able to interface in a professional manner with individuals at all levels.
3. Ability to remain calm, patient, and empathetic.
4. Must be able to adhere to the principles and mission of the Institute.
5. Ability to provide, assess, interpret, and communicate the appropriate recommendations from MMI providers to caregivers.
6. Ability to demonstrate professional behavior by adhering to departmental and organizational policies and procedures, and assuming authority appropriately.
7. Ability to be flexible, multi-task and manage competing priorities.
8. Ability to handle confidential matters discreetly.
9. Implementation of best practices, social work code of ethics related to the delivery of care coordination and case management.
