Position Summary | The MMI Care Coordination Lead Social Worker provides direct and consultative services within the Care Coordination Program to medically and socially complex patients and their caregivers receiving intensive specialty services at MMI. This role involves assessing psychological and social needs, collaborating with healthcare professionals to develop treatment plans, and providing counseling and therapy to help patients and families navigate the emotional and social impacts of illness and disability. The position is part of an interdisciplinary team that includes social workers, behavioral health providers, interpreters, medical providers, and parent resource coordinators. |

Ability to be flexible, multi-task and manage competing priorities. 8. Ability to handle confidential matters discreetly. 9. Implementation of best practices, social work code of ethics related to the delivery of care coordination and case management. |
