Details/Requirements:
Start ASAP. Emergency credentialing can be done.
3-6 months of coverage.
Monday-Friday, 8am-4pm.
Open to 2 candidates if necessary to fill schedule.
Average 8-11 patients per day.
Must have active New York license.
Active New York Medicaid is crucial, but willing to enroll while provider is working.
